A Brief Overview Accountable for direct patient care and supervision of caredelivery by ancillary staff. Coordinates the Resident Assessmentprocess to ensure that comprehensive assessments are completedtimely and that each resident has an appropriate individualizedongoing plan of care. Ensures that the staff complete initial andongoing orientation/education according to the policies andprocedures of the organization. Provides leadership utilizing theorganization’s mission. What you will do Coordinates, documents, and completes accurate Minimum Data Set(MDS) assessments. Manages the MDS schedule and assures that theMDS is completed per state and federal guidelines. Completes initial admission care plan on each resident as wellas well as updates care plans and status boards on a daily basis toaccurately reflect the residents’ status/care needs. Assures thatcare plans meet the requirements of the state and federalguidelines. Works closely with the interdisciplinary team to keep opencommunication with families and team members. Advocate for Residentrights. Leads resident care conferences. Provides nursing care in accordance with the facility policiesand procedures within the scope of practice of the RegisteredNurse. Maintains a high level of clinical competency and holdsneighborhood staff to a high standard of performance. Sharesknowledge with staff and serves as a role model. Active participant in the QAPI program. Assists with collectionand maintenance of data from OSHA and DOH. Assures nursing assistant staff is trained per policy offacility (CNA certification/medication certification) and thatcompliance with the program meets state requirements. Daily rounding with staff and residents. Completes and monitors scheduling of staff. Providessupervision of nursing team and gives counseling and positivereinforcement as needed. Responsible for overall compliance and success with the annualDOH survey process. Supports application of the organization’s mission, vision,core values, and social principles which providing quality care andservices to our patients, families, co-workers, physicians,volunteers, and the communities served. Required Education, License/Certification, or Work Experience:
Registered Nurse (RN) – Board of Nursing An active license inthe state of practice
