JOB SUMMARY

Cooperative Health is a Federally Qualified Health Center providing ambulatory primary care to patients in 19 primary care offices across 4 counties (Richland, Lexington, Fairfield, and Newberry). Our practices are credentialed as Patient Centered Medical Home (PCMH) which defines the delivery of primary care for Pediatrics, Internal Medicine, and Family Practice. PCMH is an advanced care delivery model, within a primary care practice, that adopts team-based care to engage the patient as a member of the patient care team, and when applicable the patient’s family/caregiver. The patient care team engages in continuous process improvement, increased patient access, care coordination, and comprehensive, team-based holistic care. The Clinical Nurse Educator is a registered nurse who serves as a training and administrative liaison and resource person for clinical staff members to include Certified Medical Assistants, Licensed Practical Nurses, and Registered Nurses within the Department of Nursing. The Clinical Nurse Educator provides support and guidance for adherence to policies and procedures, ensures compliance to state and regulatory agency requirements, ensures that all clinical staff members maintain professional licensure requirements; and is responsible for coordinating and developing training. The Clinical Nurse Educator will focus on nursing team members at Adult, Pediatric & Family Practice clinics (primary health and other practices/programs) as s/he provides assistance with training and staff educational development. S/he shall also support the mission, vision and values of Cooperative Health, and work towards realizing its objectives through support of teams within the department.

Nursing Education:

Coordinates and participates in employee training and determines clinical competency. Ensures that nursing staff is properly evaluated during their probationary period, annually, and periodically as needed Responsible for ensuring that newly hired employees are properly orientated to the site to include all temporary employees and student externs, with and in coordination with charge nurses and clinical practice managers Quality Management Assists with the development of policies, procedures, and standards reflective of evidence-based practice Effectively communicates new policy and procedure updates to clinical staff Uses knowledge of policies and procedures to identify and act on non-compliance Assists in rectifying and evaluating all incident reports Assists the charge nurse to resolve identified issues affecting the delivery of clinical care Ensures compliance with all infection control policies and procedures; ensures all potential infections are correctly documented and appropriate care is rendered Provides education and serves as resource for VFC Program as outlined by DHEC Performs chart audits to ensure that identified quality measures are appropriately documented Audits monthly pharmacy inspections to ensure medications in the clinical area are monitored and managed properly Coordinates and leads annual competency assessments for clinical staff in conjunction with charge nurses Provides clinical consultation, technical assistance and on-going training as necessary to clinical staff Conducts monthly and periodic BLS Instruction for all Cooperative employees Professional Growth and Development Maintains clinical competency and serves as a clinical role model by: Regular consultation with Chief Nursing Officer, Director of Nursing, clinicians, charge nurses, Clinical Practice Managers, and other team members. Attends appropriate trainings and in-service educational programs Administrative Attends regular meetings and provides updates to Director of Nursing and/or Chief Nursing Officer appropriately Serves on quality improvement committees and participates in quality improvement projects as applicable Interdepartmental collaboration with pharmacy, purchasing, Population Health, IT, finance, HIM, facilities, medical providers, and patient services as needed to help improve quality of care Ensure nursing training plans align with departmental goals, organizational strategic plan and state/federal requirements. Perform other related duties as required for the smooth running of the department

QUALIFICATIONS

Bachelor’s degree from an accredited nursing school preferred Five (5) years of experience in a medical facility, nursing home, private practice or community health center with at least three (3) years in nursing supervision, quality management, and/or nursing education Proven leadership abilities, management skills, and have knowledge of policies, procedures, quality assurance and compliance Previous experience with electronic medical records, with knowledge of statistics, data collection and Health Informatics preferred Currently licensed to practice as a Registered Nurse in the state of South Carolina Current BLS certification
