  • Central Alabama has a rich history expanding almost 50-years of enhancing the quality of life of individuals and families affected by sickle cell disease through education, advocacy, and support.

The organization was formed in 1976 by a group of concerned citizens that: (1) Recognized there was no place to contact locally for information about sickle cell disease; (2) The sickle cell test was not a standard physician-offered exam; and (3) The analysis of the test results performed often missed the detection of many clinically significant sickling disorders. The Opportunity Under indirect supervision, the Community Health Worker works closely with medical providers, primary care teams, and social services agencies to provide short term care coordination and connection to resources that support clients with sickle cell or the sickle cell trait. Expected to develop programs for clients to improve their health and general well-being through education and coordination of care and services. Works in both clinical and community-based settings, if necessary, including clients’ homes. The Responsibilities

  • Assists clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact it may have on their well-being. Help clients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
  • Documents all client encounters and contracts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.
  • Coach clients in effective management with their sickle cell disease and/or to understand their health conditions and self-care. Assists client in understanding care plans and instructions. Motivates clients to be active and engaged participants in their health and overall well-being.
  • Assists clients in accessing health related services and overcoming barriers to obtaining needed medical care and /or social services.
  • Provides support and advocacy during initial medical visit or when necessary to assure clients’ medical needs and referrals required are being conveyed. Follows up with both clients and providers regarding health/social services plans.
  • Facilitates communication and coordinate services between providers and the clients/patients. Coordinates and monitors services, including comprehensive tracking of clients’ compliance in relation to care plan objectives.
  • Assist with writing, identifying grants and other proposals to secure funding to support SCDAA for the good of the clients and organization.
  • Be available to travel to other counties served by SCDAA
  • Central Alabama to support clients at their homes, clinics/hospitals (medical facilities), community locations, various agencies, and other outreach destinations.
  • Support special events and perform miscellaneous job-related duties as assigned. The Candidate The ideal candidate will have:
  • Master or Bachelor degree in Public Health preferred, Social Work, Counseling, or a directly related field.
  • At least one year of directly related experience which may include internship and/or practicum.
  • Strong skillset and experience with supporting social media platforms and utilization of Microsoft Office Suites. Knowledge, Skills and Abilities Required
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of clients as well as constituencies in a diverse community.
  • Experience in public speaking and making presentations.
  • Knowledge of community agencies and resources.
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
  • Ability to plan, implement, and evaluate individual client care plans.
  • Knowledge of transportation and other barriers to care that may be encountered by a client.
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.
  • Skill in use of personal computers and related software applications, social media platforms, including e-mail, Microsoft Office Suites (Word, Excel, PowerPoint), etc.
  • Skill in organizing resources and establishing priorities.

  • Must pass a pre-employment criminal background check.
  • Must meet certification status required to work with sickle cell clients within 3 to 6 months of employment.
  • Employees must sign a confidentiality Non-Disclosure Agreement (NDA), Code of Conduct, and Conflict of Interest documents.
  • Must have a valid State of Alabama driver license and insured. Working Conditions and Physical Effort
  • No or very limited physical effort required.
  • Expected to support local health fairs, travel to clinics, hospitals, and other counties that fall within the Central Alabama service territory will be required.

The Relationships The Community Health Worker reports to the Executive Director who will manage the successful candidate’s daily responsibilities, caseloads, and provide semi-annual and annual performance reviews/feedback.
