ESSENTIAL JOB RESPONSIBILITIES
Attend and participate in outreach events and activities in thefield. Some non-standard work hours may be required. Identify and work with high-risk populations in collaboration withlocal stakeholders. Provide information to the community on issues of population healthsignificance. Provide navigation and linkage to access medical care Conduct or coordinate volunteer training and management to assistwith health outreach and client/community education efforts Conduct outreach visits to clients Work with identified community partner(s) to establish rapport andassist clients with linkage to care among those who have been lostto follow-up or who have never accessed care. Prepare and present education/ cultural competency sessions onspecialized topics Network & collaborate with other community organizations for LGBTQwellness, women’s health, and the empowerment of vulnerablepopulations Responsible for gathering and sharing information about issuespertaining to that specific community and building strongprofessional relationships with key community members Develop and maintain an up-to-date database of community contactswithin the community, including individuals, groups, and othersources beneficial in outreach initiatives into the community Help clients to access services by assisting in schedulingappointments, reminding clients of appointments, accompanyingclients to appointments when necessary, and follow-up on missedappointments Under the direction/supervision of the coordinator/manager, the CHWis the key between the patient and the required assistance orservices they need Communicates with patients, medical staff, and primary careprovider if necessary, to provide appropriate, timely, andcost-effective care support. Enters and maintains all demographic information, maintains allassigned databases and logs to ensure data is complete andaccurate Demonstrates positive and effective interpersonal skills whendealing with patients, visitors, peers, and other health care teammembers The liaison will provide real-time solutions to assist withpaperwork and filings, assist with telehealth medical visits, andbypass the client’s technology shortfall by offering our internetconnection or devices, to be used in the field Safety Maintains agency guidelines relating to safety, outreach andconfidentiality. Ensures proper hand washing according to Joint Commission andCenters for Disease Control and Prevention guidelines. Understands and appropriately acts upon assigned role in EmergencyCode System. Understands and performs assigned role in Health Center Continuityof Operations Plan (COOP).

JOB SPECIFICATIONS
Education:
High School diploma required.
Training and Experience:
Certified Community Health Worker (CCHW) certification orequivalent experience preferred. Completion of HIV-501 training within 6 months of hire date. Sensitivity and empathy in working with marginalizedcommunities. Knowledge base of clinical and preventive health standards of care,promoting wellness, and of acute and chronic diseases/conditionswithin the primary care setting. Performance involves weighing and evaluating factors requiringjudgment, written / analytical ability, and problem solving.
Job Knowledge and Skills:
Bilingual (English Spanish) is preferred. Computer knowledge shouldinclude Microsoft programs and familiarity with electronic healthrecords. Excellent organizational, communication, problem-solvingand teamwork skills are required. Ability to work independently andhandle stress appropriately. Ability to work cooperatively withmedical, pharmacy and other supporting staff. Ability to work withmulticultural and diverse population is required.
Contact Responsibility:
The responsibility for internal and external contacts is frequentand important. Own transportation is required. Physical Requirements This work requires the following physical activities: constanttalking in person, talking on the phone, hearing/visual acuity andexposure to blood borne pathogens and hazardous materials.
Frequent, walking, sitting and bending. Occasional standing,driving and climbing may be required. Work is performed in officesettings.
