JOB PURPOSE
The Community Health Worker (CHW) is a part of aninter-disciplinary clinical care team who helps increase access tohealth services for people through outreach, encouragingengagement, education, and peer support. The CHW is responsible for helping patients and their families tonavigate and access community services, other resources, and adopthealthy behaviors. The CHW supports providers through an integratedapproach to care management and community outreach. CHW providessocial support and informal counseling, advocates for individualsand community health needs, and provides services such as healthcare navigation. Essential Job Functions include, but are not limited to: Satisfies Core Competencies Accepts responsibility for meeting the standards of theprofessional, ethical, and relevant legislated requirements. Knowledgeable of Federally Qualified Health Center (FQHC)standards of care and requirements. Responsible for functioning according to organizationprivileges. Patient Navigation and Engagement Responsible for establishing trusting relationships withvulnerable patient populations and healthcare providers Assist patients to navigate healthcare and social servicesystems Provide ongoing follow-up, basic motivational interviewing andgoal setting with patients/families Offers community outreach and health education to individuals &groups; links community members to health & social services, andengages in inter

- Act as a patientadvocate and liaison between the patient/family and communityservice agencies Ensure cultural competence among healthcare providers servingvulnerable populations Provide culturally appropriate health education on topicsrelated to chronic disease prevention, mental health and substanceuse Address social determinants of health Documentation and Reporting Record patient care management information in the EHR and othersoftware Collect data and relay information to stakeholders to informprograms and policies
Required Education:
High School Diploma or equivalent Certification in Basic Life Support Community Health Worker training certificate must be obtainedwithin 6 months of hire Skills, Licensure, and Knowledge Requirements The ability to work with others as a team in a collaborative effortto achieve a common goal or task in and effective and efficientway.
