Community Health Worker-Behavioral Health Hours:
40hrs
Shift:
Day shift; 8:00am
- 4:30pm and 8:30am
- 5:00pm

Responsibilities Under the direction of the Director, Integrated Behavioral Health Operations, work as a member of the primary care team to expand organizational support for equitable and accessible health management of behavioral health (BH) and social determinants of health (SDOH) for all patients in a given practice(s). Provide culturally appropriate health education and utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patients plan of care. Work in a team-based role and participate in team activities such as huddles, pre-visit planning, population health management, and process improvement. Qualifications Equal to completion of 4 years of high school plus the completion of additional courses/training in community health work or behavioral health is required.
Culturally competent with solid knowledge of the communities being served.
Demonstrated ability to perform as part of a multi-disciplinary team.
Must have basic competency with documentation and computer skills.
Community Health Worker and/or Recovery Coach certification preferred, as is experience with EMR in medical settings, and Bilingual skills. Must be fully vaccinated against seasonal Influenza and the
COVID 19
virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation:
Pay rate will be determined based on level of experience. Pay Range USD $21.84
