This position works in collaboration with the Community Health and Well-Being Team, as well as the Integration Care Coordination System.

A Community Health Worker (CHW) is a frontline public health worker, who is a trusted member of and/or has a remarkable understanding of the community served. This trusting relationship with the community enables the CHW to serve as a liaison/link/intermediary between the SPHP and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. In this position, the CHW will be working a focused population of patients with uncontrolled hypertension, in an effort to improve blood pressure control, using the Screen, counsel, Refer and Follow-Up framework:
MINIMUM QUALIFICATIONS
1. High School Diploma / associate’s degree preferred. One to three (1-3 years) of community involvement experience. Ability to earn a CHW certificate is preferred. Must be able to operate effectively in a collaborative, shared leadership environment. 2. Must possess a personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health. 3. Must be able to operate effectively in a collaborative, shared leadership environment. 4. Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively engage with colleagues. Ability to effectively present information in one-on-one and small group situations to colleagues. 5. Work requires ability to communicate orally and to hear or communicate with target population. Bi-lingual skills to work with patients in their native language are preferred; certification as a medical interpreter/translator is a plus 6. Work requires proofreading and checking documents for accuracy. 7. Basic Life Support (BLS) certification is preferred. 8. Must be comfortable working and serving in a diverse and inclusive environment, and operating in a collaborative, shared leadership environment.
Pay range:
$21.50-$28.11 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
