Outreach Services Associate – Community Health Northwell Health $36,980.00 – $55,690.00 / yr United States, York, Manhasset Jun 20, 2025 Job Description Contacts and schedules patient/client’s appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports. Job Responsibility Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests.
Collects and organizes data required for statistical reports and completes reports, as required.
Attends and participates in meetings as appropriate.
Sets-up files and maintains records.
Provides incoming and in-process orientation.
Based on position, may be required to provide bi-lingual translation and/or guidance.

Fluency in English and Spanish, highly preferred.
