SHIELD Program:

The SHIELD Program is a spiritually integrated health intervention that empowers African American and Hispanic communities to prevent and manage diabetes through education, support, and advocacy.

Job Description:

The Community Health Worker will provide health education, outreach, support, and care coordination in a culturally responsive, faith-based program.

Key Responsibilities:

Facilitate health workshops and screenings Conduct outreach and support participants Build community trust and promote resilience Advocate for social and economic wellness

Qualifications:

High school diploma or equivalent Certified Community Health Worker Bachelor’s in Public Health, Social Work, Health Education, or related field Experience in African American and/or Hispanic communities Strong communication and interpersonal skills

Work Schedule:

Monday-Friday, 8 AM-5 PM, with occasional evening/weekend hours. How to
