Community Health Worker Candidates must be located in the state of Missouri The Community Health Worker is responsible for serving as aliaison/connector between the patient, care team, and thecommunity. Helps to bridge conversations with patients and removebarriers that prevent them from accessing health and socialservices. Conducts telephonic and/or face-to-face outreach to panelof patients for appointment scheduling, needs assessment, and caregap closure. How you will make an impact:

  • Meets patient in clinic, facility or at home to help identifysocial determinants of health impacting patient’s health andgeneral well-being.
  • Collaborates with social worker and/or care manager to developaction plan.
  • Develops trusting relationship with patients by providing supportand advocacy to help achieve health goals.
  • Assists patients in accessing health-related services andcommunity resources, such as accompaniment to specialistappointments and assistance with enrollment forms.
  • Facilitates communication between all parties (patients,families, colleagues, and community-based organizations) asneeded.
  • Documents interactions with patients and on behalf of patients inmedical record.
  • Attends meetings as required.
  • Travels to worksite and other locations as necessary.
Minimum Requirements:
  • Requires a high school diploma or GED equivalent and experienceutilizing electronic medical record as well as a minimum of 1 yearexperience in a healthcare, community-based, or social workenvironment; or any combination of education and experience whichwould provide an equivalent background.
Preferred Skills, Capabilities and Experiences:
  • Social work experience preferred.
  • Customer service/call center experience preferred.
  • Certified Community Health Worker preferred.
