This position is designed for individuals who are passionate about improving health outcomes and enhancing access to care for diverse populations. The CHW will engage with community members, providing education, support, and resources to promote overall well-being. Duties Serve as a liaison between healthcare providers and community members to facilitate access to services. Educate individuals and families about health resources, preventive care, and chronic disease management. Collect data on community health needs and report findings to inform program development. Assist individuals with disabilities in navigating healthcare systems and accessing necessary services. Provide support in addiction counseling, helping individuals find appropriate treatment options. Conduct public speaking engagements to raise awareness about health issues within the community. Utilize motivational interviewing techniques to encourage positive behavior changes among clients. Maintain knowledge of medical terminology and Medicare processes to assist clients effectively. Skills Proficiency in medical terminology is essential for effective communication within healthcare settings. Experience or training as a nurse or in social work is highly desirable. Familiarity with working with people with disabilities to ensure inclusive support services. Background in addiction counseling is beneficial for addressing substance use issues within the community. Strong data collection skills for assessing community health needs and outcomes. Excellent public speaking abilities to effectively convey health information to diverse audiences.
