Prevention Navigator New Breed Creation Life Center, Inc. Jacksonville, FL 32224 Overview We are seeking a dedicated and compassionate Prevention Navigator to join our team. The Prevention Navigator will play a crucial role in guiding individuals through the complexities of healthcare systems, Substance Use and Abuse and ensuring they receive the necessary support and resources. This position requires a strong understanding of public health principles, excellent communication skills, and a commitment to empowering clients through education and advocacy. Responsibilities Educate clients on available healthcare options and services to facilitate informed decision-making. Assist clients in navigating public health resources and social services, ensuring they receive appropriate care. Conduct data collection to assess client needs and track outcomes effectively. Utilize motivational interviewing techniques to engage clients and encourage adherence to health plans. Collaborate with healthcare providers, social workers, and community organizations to coordinate care for clients. Deliver public speaking presentations to educate community members about the agencies resources and programs. Maintain accurate records of client interactions and progress in accordance with organizational policies. Requirements Background in case work. Excellent public speaking skills with the ability to communicate complex information clearly and effectively. Proficiency in data collection methods for assessing client needs. Demonstrated ability to engage clients through motivational interviewing techniques. Strong interpersonal skills with a focus on empathy and client-centered care.
