DUTIES AND RESPONSIBILITIES
Under the direct supervision of the Connect Program Director of Capital Area Healthy Start Coalition (CAHSC), this position provides direct client services to pregnant and postpartum women in the prenatal care provider offices. 1. Identify, recruit, and enroll referred program participants 2. Explain the purpose of the prenatal screening tool and encourage the enrollee to complete the screening at her first obstetrical appointment 3. Provide initial intake and assessment to new clients, addressing risks and determining the appropriate individualized plan of care for each client 4. Provide ongoing care to clients determined to need further services 5. Provide education, referrals, and services to participants regarding: home visiting programs, identifying a pediatrician or family practice provider, Medicaid and insurance navigator programs as necessary, Doula supports, WIC benefits as necessary, family planning, safe sleep, community resources, prenatal care, postpartum care, Social Determinants of Health, and other risk factors, importance of early literacy and infant brain development 6. Provide information to prenatal women on signs and symptoms of preterm labor 7. Identify needs and provide referrals to available community resources as needed 8. Provide other initial client services as indicated by the Coalition 9. Adhere to timeframes in compliance with the Healthy Start MomCare Network contract, the Department of Health contract, and the Healthy Start Standards and Guidelines 10. Enter and track client information and case notes into electronic medical records database 11. Perform telehealth wellness visits with clients to ensure ongoing engagement and enrollment 12. Adhere to time frames in compliance with program requirements 13. Enter and track client data into database system 14. Reassess risks and needs during each client encounter 15. Follow up on referrals made to ensure client connection to resources 16. Attend meetings and trainings, as required, including Coalition meetings and special events 17. Act as a liaison between medical professionals, clients, home visiting programs, and our collaborative partners 18. Stay current with literature relevant to maternal and child health, such as but not limited to disparities in health outcomes, infant mortality, maternal mortality, prematurity, low birth weight, maternal infections, maternal stress, smoking cessation during pregnancy, depression, optimal birth spacing, father inclusion, social determinants of health, substance use and exposure, trauma informed care, protective factors, safe sleep, and strategies for evidence-based interventions 19. Provide outreach to the community on issues related to maternal and child health, available programs and community resources through attending health fairs and other community events 20. Provide ICC services to women as outlined in the Healthy start Standards and Guidelines and as required by current contracts 21. Create and mail provider letters 22. Participate in continuous quality improvement activities 23. Maintain confidentiality of children, families and other records and information 24. Attend meetings and trainings as required 25. Other duties as assigned by Supervisor
TRAINING AND EXPERIENCE REQUIREMENTS
Must have a minimum of a bachelor’s degree in public health, social work, health education, or related social science or health science field. At least two years of professional experience, working with clients, in social services, health care, maternal and child health, public health, or a related field preferred. Experience working with diverse communities, organizations and agencies preferred. Knowledge of pregnancy health and community engagement preferred. Strong communication and writing skills are required. Proficiency in computer applications is required. Knowledge of Florida Medicaid preferred. Spanish speaking is a plus. Must have a valid driver’s license and reliable transportation. Must successfully pass a Level II Background Screen and be able to lift 10 – 15 pounds.
