We seek to provide access to the knowledge, tools, information, and support that our citizens need to stay healthy and to begin making positive changes in their lives and in their community. The ideal candidate will have experience in public health field work and knowledge of HIV medical care and other needed services. Typical Duties + Performs case management of newly diagnosed HIV and existing HIV cases. + Provides assistance in accessing and maintaining access to HIV medical care and other needed services. + Conducts assessments and develops service plans for individuals to enter and remain in medical care. + Works to identify individuals who are “out of care,” and assists them in getting linked to care. + Maintains regular contact with clients at established intervals. + Conducts HIV medical outcome assessments, including tracking of attendance at medical appointments, and monitoring CD4 and viral load reports. + Establishes and maintains contact with clients’ HIV medical provider. + Compiles and submits statistical data and other related reports in a timely and professional manner. + Participates in coordinated EIS worker efforts within the Transitional Grant Area (TGA). + Conducts patient home visits and provider visits as necessary. Transports patients to treatment as needed. + Attends conferences and in-service programs and performs related duties. Minimum Qualifications Bachelor’s degree in the Social Sciences, Public Health, or related field Candidates with accreditations earned in a foreign institute are encouraged to apply, however, at present MPHD is not sponsoring individuals through any of the US Government’s employment-based immigration programs Preferred Experience, Knowledge, Skills, and Abilities
- Knowledge of infectious diseases and contact investigations.

- Ability to work with diverse populations and to work as member of interdisciplinary team.
