• Provide comprehensive careacross multiple settings-telehealth, in-office visits, and on-sitecare at assisted living facilities
• Ability to diagnose routineconditions and consult (or refer to) physicians for more complexdiagnoses.
• Treat patients by following best practice protocolsand consult with physician leaders if you deem it necessary tomodify the care pathways.
• Communicate with your patients asnecessary to ensure we offer a delightful experience.
• Managepatient treatment plan and provide support and education topatients on the way.
• Educate patients about Ascend’s broaderoffering, conduct health risk assessments and guide them on theirjourney to longevity and happiness.

• Master’s degree from an AMA accredited school forAdvanced Practice Providers.
• Current, active PA or NP licensurein State of employment is required.
• Internal Medicine or FamilyMedicine Board Certification (or board eligible)
• Active,non-probationary, unrestricted Georgia State License
• Active DEAlicense or eligibly to obtain
• 1+ year clinical experience ininternal medicine or primary care practice setting. Experience withgeriatric care a plus.
