The Clinical Coordinator is responsible for assisting withmanagement and operation of clinic. This position assumes fullresponsibility for the dialysis clinic in the absence of theAdministrator.
Growth:
Assist with clinical and operational processes to improvepatient health and minimize hospitalizations and missedtreatments. Assist with planning/coordinating patient scheduling to assuretimely acceptance of patients and effective staffing levels. Demonstrate effective use of supplies and staff laborhours. Responsible for updating all logs and ensuring that dialysisrun sheets and logs are sent to billing. Perform duties as assigned to meet the patient care oroperational needs of the clinic.
Outcomes:
Coordinate the functions of all departments and developstandards and methods of measuring patient care services, includinga chronological record of services provided to patients as requiredby the ESRD Network Coordinating Council and Medicare. Work with Administrator to maintain chronological, thorough,and appropriate documentation in the patient record of alltreatments, activities, and communication with the patient,physician, and other healthcare professionals. Promote quality management program through education andinvolvement of staff and patients in outcomes as well as overallresponsibility to achieve corporate goals for quality. Assist with program’s target goals for patient outcomes inaccordance with quality patient care and Company goals. Review all incident reports; make recommendations and takeaction relative to incidents as appropriate; report at monthly QAPImeeting as delegated by Administrator or Management.
OperationalReadiness:
Knowledge of and remain current with federal, state, local lawsand regulations, including health care professionals practice actrequirements. Work with Administrator to assure clinic is in compliance withall applicable federal, state, and local laws and regulations. Assist Administrator with development, implementation andfollow up of Corrective Action Plans required for internal andexternal surveys. May assume Charge Nurse’s responsibilities as needed. May fulfill responsibility of facility Alternate CEO asdelegated by Governing Body. Conduct and/or participate in both formal and informal meetingswith the governing body, Regional Director, Medical Director andthe staff as delegated. Assure that appropriate staff meetings,in-service education meetings, and team patient care planningmeetings are held monthly as delegated. Assure that QualityAssessment & Performance Improvement Program is current at alltimes as delegated. Establish, maintain, and submit all required records andreports concerning staff, patients, and the operational affairs ofthe center if delegated by Administrator. Continuously monitor to ensure that a safe and sanitaryphysical environment is maintained throughout the facility; thatall equipment is maintained and functions properly; and thatadequate and appropriate inventory levels of all supplies areavailable and used correctly. Oversee the maintenance of equipment and supplies to meetcurrent laws and regulations. Responds to all emergencies in clinic. Familiar with emergencyequipment and all emergency operational procedures. Supervise the maintenance of equipment, building areas occupiedby the center and other property belonging to the center.
Partnerships:
Maintain collaborative working relationship with MedicalDirector and physicians. Establish and maintain a positive relationship with allAdministrators, area hospitals, agencies, vendors and thecommunity. Actively promote GUEST customer service standards; developeffective relationships at all levels of the organization. Respond effectively to inquiries or complaints.
StaffDevelopment:
Ensure all staff meet required qualifications for position heldand perform duties within limitations established by and inaccordance with company policy/procedures, health careprofessionals practice acts, applicable state and federal laws andregulations. Serves as a resource/subject matter expert for patient needsand concerns, staff education and in-service sessions asnecessary. Assists with recruitment, training, development, andsupervision of all personnel. Assists with maintaining effective personnel management andemployee relations, including evaluating the performance of allpersonnel and counseling employees. Uphold management goals of corporation by leading staff in teamconcepts and promoting a team effort. Effectively communicate expectations; accept accountability andhold others accountable for performance. To perform this job successfully, an individual must be able toperform each essential duty satisfactorily.
Requirementsinclude:
Current RN license in applicable state. License must bemaintained as current and in good standing. 12 months experience in providing nursing care, including 3months of experience in providing nursing care to patients onmaintenance dialysis. CPR certification required within 90 days of hire. Confirmation of ability to distinguish all primary colors. Demonstrated ability to function in a leadership position andto perform in new and emergent situations with sound judgment. Demonstrated working knowledge of the English language andability to communicate verbally and in writing. Must have basic computer skills, including Microsoft Office(Word, Excel, Outlook); proficiency in all USRC clinicalapplications required within 90 days of hire.
