Work where you matter. A Brief Overview The dietitian isresponsible for the nutritional care of the patient within anassigned area of practice. What you will do Performs an accurateassessment within a timely manner of the patient’s nutritionalstatus including, but not limited to: obtaining and interpretinginformation in terms of the patient’s nutritional needs,patient/family level of knowledge, ability and readiness to learnabout nutrition requirements, cultural/religious needs, andphysical/mental limitations. Demonstrates knowledge of age-specificfactors and performs satisfactorily related to patient’s age,infant through geriatrics, as noted on unit specific guidelines andcompetencies. Plans the delivery of medical nutrition therapy andimplements an individualized outcomes based nutritional care planspecific to patient’s needs with clearly defined, measurable goals.
Demonstrates ability to provide medical nutrition therapy topatients/family based on assessment needs. Ensures continuity ofnutrition care through communication with relevant care providersat mission and discharge. Helps to effectively transition patientsto a new care environment. Documents all phases of the nutritioncare process in a complete and concise manner. Develops andinterprets patient education classes and/or materials. Demonstratesability to educate the public by planning and presenting lecturesrelated to nutrition therapy and community nutrition. Providingguidance to dietetic assistant in identifying and assigningpatients to a nutritional risk level using screening/assessmentpolicies and procedures. Functions as an integral member of aninterdisciplinary healthcare team as evidenced by activeparticipation in team conferences, multidisciplinary care planning,and ongoing communication of outreach contracts. EssentialQualifications The individual must be able to work the hoursspecified. To perform this job successfully, an individual must beable to perform each essential job function satisfactorilyincluding having visual acuity adequate to perform position dutiesand the ability to communicate effectively with others, hear,understand and distinguish speech and other sounds. Required Education,License/Certification, or Work Experience:
Bachelor’s Some CollegeDietetic Internship or similar Academy of Nutrition and Dieteticsapproved program required, unless already registered with theCommission on Dietetic Registration (CDR). Registered Dietitian
