Position Summary:

The Licensed Dietitian is responsible for assessing the nutritional needs of patients, developing and implementing nutrition care plans, and providing medical nutrition therapy (MNT) as part of an interdisciplinary healthcare team. The Dietitian plays a key role in managing chronic conditions such as diabetes, hypertension, obesity, HIV/AIDS, and gastrointestinal disorders through personalized nutritional guidance and health education.

Essential Duties and Responsibilities:

Conduct comprehensive nutrition assessments and individualized nutrition plans based on patient medical conditions, laboratory results, and dietary habits Provide evidence-based medical nutrition therapy and ongoing follow-up Counsel patients and families on nutritional issues and healthy eating habits Collaborate with medical providers, case managers, and behavioral health staff to integrate nutrition into patient care plans Support group education sessions for chronic disease management and wellness promotion Maintain accurate and timely documentation in the electronic health record (EHR) in compliance with HIPAA and FQHC standards Participate in quality improvement initiatives and grant-funded nutrition programs Assist in developing educational materials, outreach efforts, and community nutrition initiatives Stay current with nutrition research, FQHC best practices, and licensing standards

Qualifications:
Required:

Bachelor’s degree in Nutrition, Dietetics, or a related field Completion of a dietetic internship or coordinated program in dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) Current Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration (CDR) Licensed Dietitian/Nutritionist (LDN) in the State of Florida Minimum 2 years of experience in a clinical or community health setting, preferably at an FQHC or public health organization Proficiency in EHR systems and Microsoft Office Suite Excellent interpersonal, counseling, and communication skills Sensitivity to the cultural, socio-economic, and medical needs of diverse patient populations

Work Environment:

Full-time, on-site position Monday-Friday with occasional weekend community events Collaborative interdisciplinary team environment
