POSITION PURPOSE
Responsible for collaborating with both internal and external entities to provide nutritional and dietary counseling, education and resources to individuals, targeted populations, providers, and clinical staff on the principles of nutrition. Develop, organize and lead individual and group presentations and webinars on dietary/nutritional content. Research content and design programs, research community resources and make patient referrals.

Necessary Contacts:
All areas of the company including marketing, customer service, network innovations, provider network, clinical solutions; face to face and telephonic contact with physicians and physician administrative and senior level personnel, health industry vendors, community care providers on a regular basis.
QUALIFICATIONS
Education
Bachelor’s Degree in nutrition or related field required
Work Experience
3 years of experience as a Registered Dietitian required
Skills and Abilities
Must have ability to research and analyze member contracts/cases and make appropriate quality and cost effective decisions and referrals
Ability to work independently and make decisions required.
Must have strong communication skills including the ability to effectively explain/present information to persons with varied levels of insurance/benefits understanding.
Must have demonstrated evaluation skills at patient and populations level.
Able to travel the state as needed and lead video sessions/visits
Licenses and Certifications
Certification in a cardiometabolic disease area preferred
Current, unrestricted Food ServiceRD – Registered Dietitian license in the state of Louisiana and/or in the required jurisdictions, or where services are provided required
ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS
Coordinate, plan and conduct both individual and group onsite and tele-health session visits with members, providers and staff on the principles of nutrition.
Collaborate with all areas of the company necessary and vendors to set up programs. This will include organizing and leading group sessions to assist in development of employee and member wellness programs.
Utilizes a collaborative, coaching process to assess nutritional needs and diet restrictions to develop and implement dietary care plans and engage available resources, with joint goal setting based on readiness to change stages
Establishes working relationships with referral sources including, but not limited to, multi-specialty teams and a variety of community, state and federal resources in cross-cultural settings to ensure appropriate referrals are facilitated to address identified barrios to recommended nutritional plan.
Performs evaluation of results of both individual and population based with member while serving as an advocate and liaison with members of the treatment team to ensure optimal health outcomes for the member.
Responsible for meeting annual targets for program performance as mutually agreed to by management team to maximize program value.
Accountable for complying with all laws, regulations and accreditation standards associated with duties and responsibilities.
