Job Description

Dietitian•Diabetic Clinic•Days 3.6 3.6 out of 5 stars Baton Rouge, LA 70817 The Dietitian provides individualized diabetes self-management education and training to individuals and/or groups. Consults with hospital multidisciplinary team in coordinating care. Plans and coordinates the Diabetes Center Program of Woman’s Hospital. Maintains the knowledge and skills required to plan, coordinate, implement, and evaluate the diabetic patient. Ensures the National Standards for Diabetes Self-Management Education (NSDSME) are met and maintained at all times.

Requirements:

A bachelor’s degree in clinical nutrition, food and nutrition, or related area A master’s degree in nutrition or exercise physiology is preferred Completion of a didactic program in dietetics and a supervised practice program approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association Registered Dietitian with active registration by the Commission on Dietetics Registration of the Academy of Nutrition and Dietetics Certification/licensure as required by the state of practice Certified Diabetes Educator by the National Certification Board for Diabetes Educators is highly preferred At least three years of experience in nutrition assessment and counseling with a minimum of one year of experience in diabetes education and care. Responsibilities of the position include, but are not limited to: Obtains relevant data (e.g. physical assessment, history, diagnostic data,etc.) and communicates data to other health team members. Identifies individual needs and limitations involving patients, formulates and initiates plan of care, reviews plan, and updates plan as patient condition warrants and documents care, education, and patient response accurately, concisely and completely according to ADA standards in accordance with program requirements to assure accreditation. Implements physicians’ orders and utilizes sounds judgment to provide appropriate treatment and education by measuring and monitoring basic physiological and psychosocial functions according to ADA standards of care. Evaluates patient’s condition on an ongoing basis and responds with appropriate interventions according to ADA standards of care and services as a resource to patients upon program completion. Communicates pertinent information regarding diabetic center, patient status, problems and anticipated problems in an accurate, complete, concise, timely and courteous manner. Performs procedures with hospital equipment and instruments specific to diabetes education according to ADA guidelines. Oversees the planning, implementing and evaluation of the DSME program and participates in the evaluation of the ADA program recognition effectiveness. Provides assessment teaching and follow-up instructions to ADA Program Recognition participants. Able to educate and advise bariatric patients following bariatric clinic guidelines. Prepares patient-specific plans of care, education, and discharge notations. Documents notes in accordance with bariatric dietitian policies and procedures. Able to educate and advise patients with dietary-related concerns following outpatient general nutrition clinic guidelines. Prepares patient specific plans of care, education, and discharge notations. Documents notes in accordance with outpatient general nutrition policies and procedures. Cross-trains outpatient dietitians to ensure competency in outpatient diabetes clinic setting. Updates dietitians on changes to clinic policies and procedures and ADA standards or care. Monitors and facilitates maintenance of dietitians qualifications (CE credits, licensures, registration, etc.). Stays abreast of current diabetes care guidelines, including dietary guidelines, oral and insulin therapy, and other relevant research. Leads DPP sessions and participates in ADA DPP required activities (LSC SharePoint site, webinars, phone calls, site visits, CDC program data reports, surveys, focus groups or other evaluation activities) for each cohort. Arranges and coordinates the activities of the Advisory Group. Acts as a liaison between the staff, Advisory Group, other departments, and administration. Perform other duties assigned by Department management according to policies and procedures and the mission of Woman’s Hospital. Hours Monday•Friday;
