- Primary Purpose
- To assess the nutritional status of prenatal patients and develop a plan of care that will meet their nutritional needs and result in an optimal pregnancy outcome.##
- Essential Functions
- 45% Assess the nutritional status of all maternity clinic prenatal patients using anthropometric data, biochemical data, clinical data, dietary data, and eco-social data.
Develop a care plan for each patient based on the assessment. Assist the patient in understanding and implementing the care plan. Audiovisual materials, discussion and printed materials are used in the implementation of the care plan. Follow-up to ensure that the patient is implementing the care plan and to adjust if necessary. Reassess and modify the plan as the patients condition changes. Document the plan and patients response to the plan in Crossroads and provide feedback.30% Develop therapeutic care plans for nutritionally high risk prenatal patients. Nutritional high risk may be the primary concern, or secondary to some medical condition. Frequent risks include adolescence, diabetes, hypertension, obesity, anemia, renal disease, irregular weight gain, and substance abuse. Document the care plan in the health department electronic health record. To reassess and modify the plan as the patients condition changes. Document the plan and patients response to the plan in the electronic health record and provide feedback.20% Certify the patient in WIC if eligibility criteria are met. Document the assessment and care plan in the Crossroads system. Document the plan and patients response to the plan in Crossroads and provide feedback. Assign food packages as needed to WIC clients.5% Participate in monthly maternity staff meetings to coordinate nutrition procedures, goals, and patient care plans with other team members, i.e., physician, nurse practitioner, nurses and social workers. Present updated nutrition information that may be applied in maternal care##
- Minimum Education and Experience
- Registered Dietitian with the Commission on Dietetics Registration; or Master’s degree in Dietetics, Public Health Nutrition, or Nutrition; or graduation from a Commission on Accreditation for Dietetics Education – approved Didactic Program in Dietetics and two years nutrition experience; or Dietetic Technician, Registered with the Commission on Dietetics Registration with a Bachelor’s degree in any subject area from an accredited four year college or university and two years nutrition experience; or a Bachelor’s of Science degree in Dietetics, Public Health Nutrition or Community Nutrition from an accredited four year college or university and two years nutrition experience.
Registered Dietitian (RD) credential is required to meet program requirements.
