Position Summary:

The Nutritionist will play a key role in supporting populationhealth initiatives by providing nutrition counseling and educationas part of a multidisciplinary case management team. This positionwill focus on improving health outcomes, reducing hospitalreadmissions, and enhancing chronic disease management throughevidence-based nutrition interventions. This position will alsosupport the health and wellness initiatives provided through casemanagement to eligible UMMS employees. The role involves bothin-person consultations and virtual support for patients, ensuringaccessibility and continuity of care.

Key Responsibilities:
Patient-Centered Nutrition Care:
  • Conduct comprehensive nutrition assessments for patientsidentified through case management referrals.
  • Develop and implement individualized nutrition care plans incollaboration with the case management team.
  • Provide virtual and in-person nutrition counseling to patientswith chronic conditions (e.g., diabetes, cardiovascular disease,obesity, kidney disease).
  • Address food insecurity and connect patients with communityresources, meal programs, and benefits such as
SNAP/WIC.
Care Coordination & Team Collaboration:
  • Work closely with nurses, social workers, care coordinators,and physicians to align nutrition strategies with overall careplans.
  • Participate in interdisciplinary case conferences to discusshigh-risk patients and recommend nutrition interventions.
  • Assist in developing and implementing population healthstrategies related to nutrition, including health educationinitiatives.
Data & Quality Improvement:
  • Track patient progress, document interventions, and measureoutcomes related to nutrition support in electronic health records(EHR).
  • Use data analytics to identify trends, gaps in care, andopportunities for targeted nutrition interventions.
  • Contribute to quality improvement initiatives to enhancepatient engagement and improve clinical outcomes under value-basedcare models.

Uses knowledge and skills to identify and re-evaluate nutritionrisk throughout patients stay; independently performs the nutritioncare process, including nutrition assessment, diagnosis,intervention, monitoring, and evaluation, specific to age andmedical status. In collaboration with team, develops aninterdisciplinary nutrition plan of care for patients at high risk.

Modifies plan of care according to own professional knowledge. Discusses plan of nutrition care (e.g. nutrition support,education, etc.) with patient/and or family; provides medicalnutrition therapy as indicated; electronically documents nutritionplan of care and education provided. Provides guidance and training to nutritionists, dietetictechnicians and interns in conducting nutrition risk assignments,assessments, and in providing nutrition care to patients. Collaborates with clinical nutrition team members, as well asoutside dietitians and vendors, to facilitate smooth transition andcontinuity of care for patients, including upon hospitaldischarge. Actively contributes to teamwork and achievement of departmentgoals; attends staff meetings and may support community events.

Cooperates with peers and is flexible with cross coverage.

Interacts with others in a polite, courteous, and supportive mannerin alignment with UMMC standards of behavior.

Work Environment:

Hybrid role with a combination of remote telehealth sessionsand in-person consultations at healthcare facilities or communitysites. May require occasional travel for onsite meetings, communityevents, or patient visits. Department

  • UMMS Population Health Services Organization Reports to the Manager of Population Health Case Management Qualifications Bachelor’s Degree in Food and Nutrition, Dietetics or relatedfield is required.

Registration by the Academy of Nutrition and Dietetics,Commission on Dietetics Registration (CDR) is required. Current licensure in the state of Maryland as aDietitian/Nutritionist required. 6 months of clinical experience is required as a RegisteredDietitian. Prior completion of a

Specialty Fellowship Program or SpecialtyCertification:

e.g. CNSC, CDE, CSR, CSP, CSO, IBCLC, etc. orachievement of MS is required. Maintenance of 30 CEUs/2 years isrequired Master’s degree in Nutrition, Dietetics, or related fieldpreferred Certified Diabetes Educator (CDE) or other relevantcertification preferred.
