Sun RiverHealth is a Federally Qualified, Non-Profit Health Center servingcommunities in Suffolk, Rockland, Orange, Dutchess, Ulster,Sullivan, Columbia and Westchester County. Sun River Health iscurrently seeking a full-time Dietitian to join our team in theBronx. The ADHC Dietitian plans and delivers nutritionalinformation to assist in the promotion of health and control ofdisease.

Essential Responsibilities and Job Duties:

Plans all menuswith food services to meet the various nutritional needs of theclients. Conducts in service/presentations to clients enhanceunderstanding of the role of nutrition in health and promote therole of the dietitian. Maintains confidentiality of all pertinentclient care information to ensure clients are protected. Assessesclient nutritional status and needs according to facility andregulatory agency standards, and participates as part ofinterdisciplinary care planning. Evaluates medical records toobtain necessary medical information to determine clientnutritional status, needs, and develop nutrition care plans.

Consults with physicians, nurse practitioners on appropriatenutritional intervention to improve client nutritional status.

Provides nutritional counseling and education for clients to assistthem in making desired changes in eating habits. Coordinates allnutritional necessities for client services with otherinterdisciplinary team members in the comprehensive care planningprocess. Provides drug-nutrient interrelationship information toassure that clients receive maximum benefits from drugs and diet.

Maintains accurate records to meet regulatory agency guidelines andassists clients in making desired changes in eating habits.

Completes all initial and quarterly assessments within theappropriate time frames. Attends and participates in seminars andcontinuing education programs to expand knowledge of current trendsin nutrition and in issues/ concerns of the

population.

Attends all mandatory in-service programs including fire safety,infection control, clinical

manifestations, transmission,prevention psycho-social issues, confidentiality and client rights.

Maintains confidentiality of all pertinent client care informationto ensure residents are protected.

CertificationSalary Range:
