We believe food is medicine and that access to nutrition is key to equity in health outcomes. Position Summary We are seeking a dedicated and culturally competent Nutritionist to join our team. This role will support individuals and families in achieving better health through personalized nutritional assessments, education, and food access solutions including Medically Tailored Meals, food prescriptions, and community pantry programs. The Nutritionist works closely with participants to assess their nutritional needs, provide health coaching, and implement dietary strategies that promote overall well-being. The Nutritionist will play a crucial role in supporting participants through various health challenges, including weight loss and acute care situations. Key Responsibilities Conduct comprehensive nutritional assessments and develop individualized care plans Provide culturally appropriate nutrition education and counseling Coordinate services such as meal delivery, pantry stocking, and access to food-related benefits Collaborate with care managers, healthcare providers, and community partners to ensure seamless wraparound support Track participants outcomes and contribute to continuous improvement initiatives Participate in outreach events and community engagement to build trust and awareness Provide health coaching to empower patients in making informed dietary choices. Maintain appropriate and timely services documentation as per SOMOS-SCN and organizational policies. Stay current with research and best practices in nutrition and dietetics. Qualifications Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field Licensed Dietitian/Nutritionist (LDN) or Registered Dietitian (RD) credential preferred Strong knowledge of physiology and its application in nutrition science. Experience in patient assessment and developing individualized care plans. Experience working in community health, social care, or food access programs Bilingual or multilingual skills a plus, especially in Spanish Passionate about social justice, health equity, and culturally sensitive care Excellent communication skills for effective health coaching and patient education. Ability to work collaboratively within a healthcare team, particularly in acute care settings. Passion for promoting healthy lifestyles and improving patient outcomes through nutrition.
