SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES

Perform nutrition assessments and reassessments daily as per assigned unit and in order of risk level, admission date and time as per the current policy of the department.
Visit patients to obtain a detailed nutrition history summary and determine their nutritional needs. Complete comprehensive assessments and communicate recommendations to the physicians and/or nursing staff.
Perform meal rounds to observe patient’s acceptance of meals. Obtain patient food preferences and allergies and communicate them to the kitchen diet office.
Provide nutrition education and counseling to patients and/or family/caregivers identified at nutritional risk prior to discharge. Education is to be reflective of the patient’s nutritional needs and in accordance with therapeutic diet guidelines.
Participate in discharge planning rounds and interdisciplinary team rounds in assigned unit to discuss the plan of care and recommendations with the medical team.
Document all assessments, reassessments, likes, dislikes, allergies and all other pertinent information in the electronic medical record.
Document daily productivity of all patients assessed and/or visited and any patient-associated meetings daily. Provide daily productivity reports to the clinical nutrition manager on a weekly basis.

ADDITIONAL RESPONSIBILTIES

Attend and participate in department monthly meetings.
Conduct tray assessments and evaluations as per schedule provided by the department.
Assist with orientation and on the job training of newly hired dietitians or students.
Conduct scheduled group nutrition classes on different nutrition topics on behavioral health units.
Collect data, participate in department QA, and PI projects; can be asked to develop and coordinate ongoing quality assurance programs by implementing performance indicators and pursuing corrective actions to ensure adherence to established standards.
Assist CNM in review, planning and updating of menus for the general population as well as for therapeutic diets.
Participates in the formulation and review or departmental and hospital wide policies and procedures for the operations of the department.
Attend continuing education programs to keep abreast of new guidelines in the nutrition field
Perform additional duties as deemed necessary for the department.

Minimum Qualifications Qualification Requirements:

For appointment to Assignment Levels III and IV
1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility:
Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
