Non-Exempt Position Summary:

CABS Health Network is seeking a compassionate and detail-oriented Nutrition Assistant to join the Enhanced Services Department. Reporting to the Program Manager – Enhanced Services in the interim, this team member will operate at the Patient Navigator level, supporting the Nutritionist/Dietitian in delivering high-quality nutrition interventions to Medicaid members and high-need populations. The Nutrition Assistant will play a key role in tracking meal deliveries, monitoring client satisfaction, supporting outreach and retention efforts, and ensuring coordination of follow-up visits. By serving as a critical link between the nutrition team, clients, and vendors, the Nutrition Assistant ensures that nutrition services are accessible, impactful, and client-centered.

Key Responsibilities:

Client Navigation & Support Serve as a consistent point of contact for clients receiving medically tailored meals or nutritional support. Conduct 7-day and 30-day follow-up calls with clients to evaluate satisfaction, identify challenges, and assist. Support client onboarding and assist with scheduling sessions with the Nutritionist. Meal Delivery & Quality Tracking Track and confirm all meal deliveries through vendor systems and client outreach. Record client-reported satisfaction, delivery issues, and feedback on meal taste, quality, and health impact. Ensure issues are escalated to the Program Manager or Nutritionist for timely resolution. Coordination with Nutrition Services Manage scheduling and reminders for nutrition counseling appointments. Maintain updated logs on dietary preferences, restrictions, and health indicators. Assist the Nutritionist in preparing handouts, follow-up plans, and educational materials for clients. Outreach, Retention & Engagement Conduct outreach calls to educate eligible clients on available nutrition services. Re-engage inactive clients and support re-enrollment into nutrition and meal services. Coordinate closely with care managers and CHWs for client referrals and follow-up.

Minimum Qualifications:

Associate Degree in Nutrition, Public Health, Human Services, or related field. At least 3 years of experience as a Patient Navigator , Health Advocate, or Care Coordinator. Minimum 1 year of experience supporting a nutrition or food-based intervention program. Excellent verbal and written communication skills, with strong follow-up ability. Familiarity with Medicaid populations and culturally diverse communities. Proficient in Microsoft Office (Word, Excel), EHR systems, and tracking software. Bilingual skills (Spanish, Haitian Creole, Russian, Mandarin/Cantonese) are highly desirable
