SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES
Perform full nutrition assessments including, but not limited to the following: Gather nutrition history, medical history, laboratory data, medications, and meal patterns, and other relevant data. Assess patient for nutrition issues. Define an individualized treatment plan and develop related patient goals, and Monitor and evaluate the progress each patient is making toward established goals and expected outcomes. Demonstrate knowledge of safe and sanitary practices and principles of dietetics and nutrition. Accurately and comprehensively document results of nutritional assessments and client counseling in the medical record in accordance with department policy, Joint Commission, OASAS, DOMH and CMS guidelines. Demonstrates knowledge of proper procedures involved in the development of therapeutic or special diets. Provide individual or group nutrition education classes for patients using approved materials and/or curriculum. Provide counseling and nutrition education to patients and caregivers as appropriate Demonstrates competency in performing required calculations to determine proper nutritional amounts. Demonstrates skills in educating clients and caregivers regarding proper nutrition, food selection and preparation. Support interdisciplinary performance improvement goals of the Substance Use Disorder department. Attend and participate morning and afternoon huddles, unit meetings, monthly staff meetings, and team meetings. Complete and maintain records of weekly or monthly productivity reports as assigned. Assists in collecting data for quality assurance purposes as appropriate and required. Develop population-specific nutrition education materials as needed. Participate in outreach activities and maintain liaison with community organizations Actively pursue continuing education and career development programs for professional growth. Be aware of National Patient Safety Goals and consistently enforce Hospital policies. Be aware of Chemical Dependency treatment modalities as they relate to a patient’s nutritional status. Maintain positive and professional demeanor with patients, visitors, co-workers, and hospital administration. Perform any additional duties and adapt to changes in work load as assigned by the supervisors defined above.
Minimum Qualifications Qualification Requirements:
For appointment to Assignment Levels III and IV 1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility: Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire. Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
