No County Queens Street Address DOH/NYS Veterans Home at St Albans 178-50 Linden Boulevard City Jamaica State NY Zip Code 11434 Duties Description Provides medical nutrition therapy, including nutrition assessment, diet modification, nutrition education, and intervention for the patient population

  • Follows all guidelines as outlined within the State Diet Manual and the Clinical Nutrition Policies & Procedures Manual.
  • Complies with all regulatory standards, including federal, state, and accrediting agencies, while adhering to facility confidentiality, HIPAA regulations, and patient rights policies
  • Trains and mentors staff as applicable
  • Participates in patient satisfaction programs, departmental meetings, and facility-wide Quality Assurance/Performance Improvement programs. Establishes goals and oversees implementation of clinical nutrition care based upon the facility’s medical direction and patient population
  • Assesses the nutritional status of patients through documentation in the medical record.
  • Leads, directs, coaches, trains, and develops nutrition team members.
  • Trains and mentors patient services staff and interns as applicable
  • Fosters teamwork and establishes effective strategies to implement innovative programs leading to optimal clinical nutrition and financial outcomes.
  • Complies with regulatory standards, including federal, state, and accrediting agencies, while adhering to facility confidentiality, HIPAA regulations, and patient rights policies.
Minimum Qualifications Reassignment:

Current New York State Department of Health Veterans’ Home at St. Albans employee with permanent or contingent permanent status as a Dietitian 2Candidates from outside and within State Service may be considered for hire under the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program as a Dietitian 2 if they have:

Current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD) or New York State Education Department (NYSED) certification in dietetics-nutrition and have completed a minimum of an additional 400 hours of supervised dietetics practice under the supervision of a RD.

Transfers:

Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service in a position that is eligible for transfer under Section 70.1 of the Civil Service Law to Dietitian 2.

Promotion Provisional Appointment:

In the absence of an eligible list with three or more candidates willing to accept a position, a temporary provisional appointment may be considered. For provisional appointment, candidates must meet the following minimum qualifications: Be a current NYS Department of Health employee with one year of permanent competitive or 55-b/55-c service as a Dietitian 1 and have a current registration with the CDR as an RD or certification as a Dietitian-Nutritionist by the NYSED. Additional Comments Work week Tuesday through Saturday off on Sunday and MondayOne competitive full-time appointment, or non-competitive full-time appointments via HELPS.
