We are seeking a dedicated and knowledgeable Dietitian to join our healthcare team. The ideal candidate will play a crucial role in promoting health and wellness through nutrition. You will work closely with residents to assess their dietary needs, develop personalized care plans, and provide guidance on healthy eating habits. This position requires a strong understanding of nutrition science, patient assessment, and the ability to effectively communicate dietary recommendations. Duties Conduct comprehensive resident assessments to evaluate nutritional needs and dietary habits.
Develop and implement individualized care plans that address specific health goals, including weight loss and management of chronic conditions.
Provide health coaching to residents, guiding them towards healthier lifestyle choices.
Perform diagnostic evaluations to identify nutritional deficiencies or imbalances.
Collaborate with healthcare professionals in long-term healthcare settings to ensure integrated resident care.
Supervise dietary staff as needed, providing training and support in nutrition-related practices.
Stay updated on current nutrition research and trends to provide evidence-based recommendations.
Educate residents on the importance of nutrition in managing their health, particularly in geriatrics or other specialized populations. Skills Strong knowledge of nutrition science and dietary guidelines.
Proficiency in resident assessment techniques and diagnostic evaluation methods.
MDS assessments completions
Experience in developing effective care plans tailored to individual needs.
Ability to coach residents on healthy eating habits and lifestyle changes.
Excellent communication skills for educating residents and collaborating with healthcare teams.
Familiarity with systems used for tracking resident progress and outcomes related to nutrition.
Understanding of geriatric nutrition needs and common aliment-related issues within this population.
Attending interdisciplinary team meetings to best meet the holistic needs of each resident.
Specified weight management goals for each resident.
