Senior Director/AVP, Chronic Care Mgt & Transitions (Medicare, Nursing Leader) 3.6 3.6 out of 5 stars 1010 Northern Boulevard, Great Neck, NY 11021 Fast-paced, rapidly growing primary care organization seeks high energy, super organized, multi-tasking clinical leader to direct our emerging care management team. Our company was founded on the value of the doctor-patient relationship, especially primary care. Our physicians provide extraordinary care to their adult patients through their participation in the Medicare Shared Savings Program where we rank in the nation’s top 2% of over 450 ACOs. We also manage Long Island’s 1st primary care-adult medicine medical group with over 15 sites and growing. A key element of our success in value-based programs is of course supporting our Medicare patients in-between doctors’ visits; in transitions from ER, inpatient or rehab to home; and through ongoing support through CMS’ Chronic Care Management and Advanced Primary Care Management programs . We are seeking a clinical leader to work with our small team of RNs and Patient Advocates to further develop and expand our program and create the operational infrastructure to ensure its success and ability to grow. Our RNs are assigned to specific primary care physicians and their patients to ensure continuity and the formation of personal relationships for telephonic care management. Our next stage of evolution is a hybrid of on-site and telephonic. We are seeking a leader to help us create the program, including services provided on-site, identification of eligible patients, workflows, job descriptions and KPIs, goals of the program (clinical outcomes, reduction in cost to the Medicare program and improved patient satisfaction and most importantly patient activation in their own health). The position is based in Great Neck with onsite time in the doctors’ care centers throughout Nassau & Suffolk, especially as we initially launch the new program and you oversee the rollout. Qualifications Bachelors in related field; Masters preferred ( clinical degree highly preferred – RN/NP ) Innovator with vision of how healthcare can best be delivered to an aging population, and the heartfelt desire to build a better model 10+ years experience working in care management or another clinical role; non-clinicians that manage similar programs (value-based models for example) will be considered, but administrative RNs strongly preferred Strong knowledge of chronic conditions and evidence based clinical guidelines Strong operational skills to develop workflows, policies & procedures, KPIs and reporting Superb communication skills, both oral and written Ability to use data for organized and efficient decision-making, including measuring ROI Ability to innovate to build a better model within the regulatory structures Passion to build and innovate and evangelize the model to doctors, colleagues and Medicare patients (You love what you do and work never feels like work, but rather a mission!)
