LifeStream is a behavioral health and social servicesanization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando counties.

Position Overview:

We are seeking apassionate and dedicated MA/ MS Licensed Therapist to join our team. The MA/MS Licensed Therapist will developprehensive biopsychosocial assessments clinically responsive to age-appropriate patient/family needs and concernsmensurate with knowledge in the substance abuse/mental health field.

Essential Functions and Responsibilities:

Complete psychosocial assessment on each consumer within designated time of admission. Collaborate with other professionals to evaluate consumers’ medical or physical condition and to assess needs. Provide documentation of clinical and case management services in accordance with department standards. Participate with the interdisciplinary team in developing and implementing clinical treatment plans for consumers. Serve as consumer advocate for needs within themunity. Maintain current all staff development requirements for this position. The ability toply with the regulations requiring a workplace of dignity and respect and equal employment opportunitypliance. The ability to work collaboratively with others and to accept constructive criticism from supervisors and managers. Willingness to abide by thepany’s published rules and regulations.

EDUCATION & EXPERIENCE

Education:

Master’s degree in clinical or counseling psychology, Counselor Education, Social Work, Marriage and Family Therapy, or other relevant Human Service Field required.
Licensure required as Clinical Social Work, Mental Health Counselor, or Marriage and Family Therapist valid in FL.

Experience:

Minimum of Two (2) years of direct clinical experience preferable in a health or mental health setting with an emphasis on population to be served in the assigned area.

Driving Requirements:

Must be Over the age of 21 required.
Minimum of 5 years driving experience required.
Possess a Valid FL Driver’s license (within 30 days of hire if currently do not possess)
Possess a continuously valid US Driver’s license for the past three years, from date of issue required.
Safe Driving record required.
Must be able to pass a Level II Background Screening
Must be able to pass a preemployment drug test.
