Job Description:

JOB RESPONSIBILITIES

Conducts individual and group therapysessions to educate patients regarding psychological, emotional, orsubstance abuse problems. Displays active involvement in treatmentplanning process. Provides family session counseling to allpatients to ensure appropriate communication and involvement offamily members and support groups. Actively communicates withclients, families, and outside referral sources. Demonstratesproactive communication with those involved with the patient’streatment. Works well with the interdisciplinary team includingphysicians, utilization review and nursing staff members.

Demonstrates active communication with team members. Effectivelycommunicates to UR on treatment and LOS issues. Completesappropriate paperwork for clients in a timely, accurate andcomplete manner. Demonstrates appropriate crisis intervention andde-escalation skills. Acts as member of team with crisis situationoccurs, assisting others to ensure patient safety. Completesaccurate assessments of patients utilizing clinical skills. Assistscare coordination department as requested. Completes alldocumentation thoroughly, timely, and in accordance with hospitalstandards. Displays a thorough knowledge of confidentiality.

Completes releases appropriately. Demonstrates knowledge of HIPAAregulations and abides by those standards. Demonstrates a workingknowledge of all clinical regulations, including TJC, Medicaid, andstate and federal laws. Displays active involvement in thedischarge, transition, and after-care planning treatment processesOther duties as assigned

Qualifications Education:

Master’s degreein social work or counseling or relevant state licensure required.

Experience:

Prior experience with psychiatric and chemicaldependency patients.

License:

Current unencumbered clinical licenseper state of practice guidelines.

Additional Requirements:
