Responsibilities:
Interventionist will provide direct home-based interventions to address risk factors, increase protective factors, and ensure the safety of families. Provides home-based clinical treatment utilizing the Functional Family Therapy
- Therapeutic Case Management (FFT-TCM) model to assigned caseload of up to 10 cases and provides coverage for peer’s caseload as needed Manages the extensive travel requirements involved in serving families in their homes.
Conducts assessments including intakes, review of referral information, identifying and engaging key participants, identifying relational patterns, and individualized treatment planning. Engages families to sign for services and completes intakes with families within the required ACS timeframes. Participates in all required agency, model and stakeholder trainings, individual and group coaching and staff meetings. Adheres to model expectations and implement with fidelity including the timely completion of progress notes, session planning guides, session impact scales and appointment notes in the Care4 documentation system. Actively participate in weekly consultation and supervision and site visits when they occur. Consistently meets all program and model Key Performance Indicators (including session pacing, required casework contacts, successful client outcomes, timely completion of Family Assessment Service Plans, data entry in all systems, etc.). Identify and address family risk factors that give rise to possible child removal, ACS involvement, and out of home placement due to continued referral behavior or new risk/safety factors. This includes the completion of safety plans and incident reports if necessary. Maintains clear, concise and timely documentation of treatment efforts in progress notes and Family Assessment and Service Planning (FASP) that demonstrate compliance with the FFT-TCM Model and supervisory review feedback. Education:
Sociology, Social Work, Physiology Bachelors Organizational expectations include: Regular and punctual time and attendance. Education Required:
