Our Brands Responsibilities:
The Family Centered Treatment Therapist provides qualitypsychotherapeutic services to children, adolescents, and familiesutilizing the Family Centered Treatment model. Provide FamilyCentered Treatment (FCT) Clinical Services as per the servicedefinition and model fidelity requirements. FCT Certification isrequired and must be completed within one year of employment inorder to continue to provide FCT services and maintain employmentwith Health Connect America (HCA). Complete FCT Checkoffs and otherinternal trainings within designated deadlines. Trainings areassigned and monitored through an e-learning system. Deliver FCTclinical services to children, adolescents, and their families,primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families andchildren. Service is regularly provided in the home and communityand will require service provision at the convenience of the childand family requiring a flexible work schedule. Complete allrequired Clinical Records and FCT documents with the child andfamily and ensure the documents are entered/uploaded timely in theElectronic Health Record, according to company standards. DevelopService Plan in coordination with Treatment Team Process andmaintain the plan per designated authorization processes in theclinician’s area of service. Work collaboratively on behalf of thechild and family with the identified Treatment Team members toensure effective communication and coordinated service provision.
Provide psychoeducation as indicated in the Service Plan. Conductinterventions with the family aligned with the FCT model to includefacilitating the movement of the child and family through the fourphases of treatment and completing all fidelity documents. Assistthe families and children to establish weekly goals and help themto identify the steps that need to be taken to reach theestablished goals. Develop and coordinate discharge plan based ontreatment, indicating all relevant aftercare needs and plans.
Access community resources that would be beneficial to clientaftercare needs. Coordinate and oversee the initial and ongoingassessment activities. Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care andpsychiatric) and non-medical (e.g., Dept. of Child Services,school, Dept. of Juvenile Justice) providers; engage community andnatural supports and include their input in the person-centeredplanning process. Provide and coordinate behavioral health servicesand other interventions for the youth or other family members withother behavioral health professionals and Treatment Team members.
Qualifications:
Minimum of a Master’s Degree in human services orrelated field is required. Two years’ experience working with youthor families. Current Family Centered Treatment Certification ispreferred.
Additional state specific requirements include:
NC, VA-QMHP or ability to register as a QMHP MS- Must be provisionallycredentialed as a PCSS in Mississippi with eligibility to be fullycredentialed as a CCSS in Mississippi. Be Well with
