We currently serve over 300 SkilledNursing Facilities including sub-acute, long-term care, andassisted livings throughout NJ, NY, PA, and MD. Key Responsibilities:

• Conduct individual and grouppsychotherapy sessions for patients in a subacute or skillednursing facility.
• Assess, diagnose, and treat a wide range ofemotional and behavioral health conditions, including depression,anxiety, adjustment disorders, and cognitive decline.
• Develop andimplement evidence-based treatment plans tailored to the needs ofeach patient.
• Collaborate with facility staff, interdisciplinaryteams, and family members to support patient care.
• Maintainaccurate and timely clinical documentation in accordance withregulatory standards using our clinician-designed electronicmedical/health system (EHR).
• Monitor patient progress and adjusttreatment plans as needed.

Requirements:

• Active and unrestrictedlicense to practice as a Licensed Marriage and Family Therapist inState applying for.
• Effective oral and written communication inEnglish
• Basic proficiency with technology, including electronichealth records (EHR).
• Strong organizational and documentationskills, with attention to regulatory compliance.
• Ability toprovide in-person services at assigned facilities, up to 45minutes.
• Ability to walk, stand, and move between patient roomsand offices within the facility throughout the day.
• FullVaccination and Booster Status may be required in some facilities.

Preferred Qualifications:

• Previous clinical experience in asubacute, long-term care, or geriatric healthcare setting.
• History of treating Anxiety, Depression and Adjustment Disorders.
• Psych or Addictions experience is a plus.
• Excellent timemanagement and organizational skills.
