The OnTrackHamilton Master’s ClinicianDuties:

Perform quality work withindeadlines with or without direct supervision.

Interactprofessionally with other employees, customer and vendors.

Workindependently while understanding the necessity for communicatingand coordinating work efforts with other employees andorganizations.

Conducts individual, family, and group counselingIsresponsible for maintaining a designated amount of direct servicetime with clients and a designated amount of contacts for theassigned caseload.

Participates in the initial individual programplanning process with all service providers (i.e.,Outreach/Education specialist, SEES, and medical staff) andprovides ongoing information regarding client progress to theservice providers.

Completes an individual treatment plan with eachclient within 30 days of enrollment.

Writes progress notes aftereach service contact with a client in accordance with the Center’sprocedure on Time Frames for Clinical Records and RelatedDocumentation.

Maintains documentation of a six-month service updatein the clinical record.

Participates in the development of acrisis/safety plan for priority clients.

Participates in regularlyscheduled Peer Review.

Follows all guidelines outlined in the HRMCPolicies and Procedures Manual including Corporate CompliancePolicies.

Is expected to have regular and predictable attendance andthe ability to work cooperatively with others.

If licensed, signingthe non-licensed clinicians’ treatment plans and being availablefor clinical consultCoordinates and schedules ongoingoutreach/referral meetings with community providers as part of theOnTrack model.

JOB PURPOSE/SUMMARY

Summary of role of team: Clinicianworks in a team format to meet the individual needs of clients inthe program, while providing clinical recommendations fortreatment.

Functions as a member of a treatment team to plan,implement, and evaluate successful interventions for individualsand families.

Summary of position:

Serves as the primary clinicianthat provides individual, group, and family counseling as part ofthe First Episode Psychosis/On Track TN model. Clinician providesweekly individual, group, or family psycho-therapy to adolescentsand young adults ages 15-30 with a mental health diagnosis thatinvolves symptoms evident of psychosis. Clinician will help clientsclarify goals, cope with stressful situations, interact moreeffectively with other people, and generally to overcome barriersto client’s recovery.

Clinician is to provide psycho-socialassessments for clients and provides diagnosis and appropriatereferrals for clients assessed, as needed. Clinician providescrisis intervention and emergency services as needed. The primaryclinician will rotate in a 24 hour/7 day a week on-call schedule.

Treatment modalities used by Master’s level clinicians providingtherapy services include Dialectical Behavior Therapy (DBT),Cognitive Behavior Therapy (CBT), CBT for psychosis (CBTp),Trauma-Focused Cognitive Behavior Therapy (TF-CBT), Attachment,Self-Regulation and Competency (ARC). Clinician embraces the keyvalues of clinical services: empowerment, normalization,rehabilitation, and continuity of care. Provides administrativeduties for the OnTrack TN program including supervisoryresponsibilities, training of new staff members and continued staffdevelopment, monthly reporting in house and to the state ofTennessee (TDMHSA), and the promotion of the program with outsideagencies. Conducts administrative and clinical supervision to acoordinated specialty care team.

JOB DUTIES/RESPONSIBILITIESQualityCare Provides individual/family psycho-therapy for youth and youngadults ages 15-30. Clinician is present and ready to begin sessionat the appointment time, every session. Clinician calls toreschedule any sessions that he/she will be unable to attend orthat the consumer missed or canceled 100% of the time. Cliniciancommunicates accurate and thorough information to outside agencies100% of the time when clients need different or more appropriatecare that HRMC cannot provide. Ensures therapy contacts arecompleted each month, according to program standards.

Providesdiagnostic Psycho-Social Assessments Clinician completes intakesand assessments within 5 business days of contact.

Assessments willbe completed in coordination with Assessment Team and ClientBenefits as evidenced by communicating any issues concerningclients or assessment.

Documentation requirementsEnsures all familyand client participation in treatment planning as evidence bysignatures on tx plans.

Communicates to supervisor if referralsbecome low in weekly supervisions.

Meet weekly with directsupervisor to discuss clinical and administrative concerns/changesCreate and implement treatment interventions that arereflected in progress note documentation based on the treatmentplan goalsRespond to all flags and e-mails within 2 businessdaysRespond to all voice mails within 30 minutesComplete allrequired clinical documentation in accordance with Center Policyand Procedure and funding source guidelines.

Update treatment plans,crisis plans, DLA-20 and updated admissions signature page every 6monthsComplete and submit productivity summaries to supervisor bythe 3rd business day of the month.

Create daily progress notes andsign in EMR within one week of service providedLeadershipAttendsand is timely to weekly multi-disciplinary team meetingsIflicensed, signing the non-licensed clinicians’ treatment plans andbeing available for clinical consultAttends weekly staff meetingswith team.

Ensures update treatment plans, consumer strengthassessments, crisis plans, consent to contact, releases, ROI,MIRECC and updated admissions signature page every 6 months asevidence by quarterly quality record reviews for OnTrack TNclientsComplies with plan to ensure tracking system for Tx plansfor all superviseesComplies with all required data for funding istracked monthly and quarterly, as requested by supervisor.

Work withTeam leader to develop and implement an ongoing plan for communityoutreach to educate and connect others to the FEP model oftreatment.

Attend trainings to advance skills in the FEP model, andensure cross-training of all appropriate roles, as part of theCoordinated Specialty Care (CSC) model.

QUALIFICATIONS – .50

OnTrack HamiltonMaster’s ClinicianExperience /

Knowledge:

Master’s degree in HumanServices or related field. One-year experience in the social workfield or a behavioral health setting preferred. Knowledge ofetiology and treatment of mental illness, physical and sexualabuse, suicide, human development, and cultural diversity.

Education/ License:

Master’s degree in human services or related field withcourse work or experience in the areas of cultural diversity, humandevelopment, etiology and treatment of mental illness, alcohol anddrug abuse, physical and sexual abuse, suicide, and developmentaldisabilities.

Physical/Emotional/Social –

Skills/Abilities:

Exposureto biological hazards. Hearing of normal and soft tones. Close eyework. Valid driver’s license- F endorsement required for thetransportation of clients. Lifting up to 50 lbs. Pushing/pulling upto 150 lbs. frequent sitting, standing, walking, bending, stooping,and reaching. Required to be certified in, and effectivelyimplement, verbal and physical de-escalation techniques thatinclude a wide range of bodily movements; including but not limitedto, grasping, holding another person, going down on knees, running,and walking.
