This role is essential in providing high-quality care and support to individuals in various settings, including assisted living facilities, nursing homes, and home community care environments. The ideal candidate will have a strong background in caregiving and patient monitoring, ensuring the well-being of those under their care. Duties Assist clients with daily living activities, promoting independence and dignity. Monitor patients’ vital signs and report any changes to the healthcare team. Provide companionship and emotional support to clients, fostering a positive environment. Assist with heavy lifting as needed for patient transfers or mobility support. Ensure safety protocols are followed while caring for individuals with disabilities. Collaborate with other healthcare professionals to develop and implement care plans. Maintain accurate records of patient care activities and observations. Skills Experience in assisting individuals in assisted living or nursing home settings. Strong caregiving skills with a focus on patient monitoring and support. Knowledge of CPR techniques and emergency response procedures. Ability to perform heavy lifting safely and effectively. Familiarity with working with people with disabilities, demonstrating patience and understanding. Excellent communication skills, both verbal and written, to interact effectively with clients, families, and team members. All QDDPs will have at least one year of experience working directly with individuals with intellectual or other developmental disabilities and will complete QDDP Relias training offered by ADMH.
