This role is centered around empowering individuals to gain and maintain the skills necessary for daily living, self-advocacy, and full community integration.
Primary Responsibilities:
Skill Development :
Coaches assist clients in learning and practicing essential life skills such as personal hygiene, meal preparation, home maintenance, shopping, money management, using public transportation, and time management.
Goal Planning and Implementation :
Each client has a personalized support plan developed in coordination with the individual, their family, and other professionals. The Supported Living Coach helps implement this plan and regularly assesses progress toward established goals.
Promoting Independence :
Coaches focus on teaching, modeling, and encouraging clients to make informed decisions, problem-solve, and advocate for themselves in various settings, including healthcare, housing, and employment.
Community Integration :
Encouraging and facilitating participation in social, recreational, and vocational activities that promote inclusion and build a sense of belonging within the community.
Documentation and Reporting :
Coaches maintain accurate and timely records, including daily progress notes, incident reports, and updates on client goals, which are critical for monitoring outcomes and ensuring quality care.
Coordination of Services :
They may work closely with other support staff, healthcare providers, families, and case managers to ensure continuity of care and alignment with the client’s needs and preferences.
Training and Requirements:
All Supported Living Coaches undergo comprehensive training before providing services.
Training typically includes:
CPR and First Aid Certification HIV/AIDS and Infection Control Zero Tolerance and Abuse Reporting Medication Administration (if applicable) Behavior Management and De-escalation Techniques Confidentiality and HIPAA Compliance Person-Centered Planning Ongoing annual training to stay current with best practices and regulatory updates Coaches are expected to demonstrate patience, empathy, strong communication skills, and cultural sensitivity. They must be flexible, reliable, and committed to upholding the dignity and rights of the individuals they support.
Additional Information:
Must pass a Level II background screening. Valid driver’s license and reliable transportation may be required depending on the nature of services provided. Experience in caregiving, social work, or related fields is often preferred but not always required.
