Summary:
The incumbent in this position is responsible for providing patient education and coordinating and linking pregnant women to care, support and services that willplement and enhance prenatal care to promote their health and their baby’s health.
Duties and Responsibilities:
Recruit participants to conduct Initial Intakes with referrals and enroll participants into MamaCare Rising services from all 4 clinic locations.
Identify participant’s risk factors, concerns, priorities, strengths and resources.
Conduct screening to identify risk factors such as domestic violence, substance use/abuse, perinatal maternal distress and/or mental health issues.
Enroll women with substance use disorder, mental health or developmental delays into Plan of Safe Care.
Make referrals to appropriate resources as needed such as WIC, etc.
Provide education to participants utilizing program materials to reinforce care plan regimen implemented by medical care team, promotion of medicalpliance and progress on medical goals.
Assist participants with accessing medical care and monitor the effectiveness of services and adjust care plan as needed.
Ensure that each participant schedules andpletes postpartum visit.
Collect and enter data into the designated data base and participate in quality improvement activities as directed.
Provide disease and chronic illness management education, referral and follow-up services, including referrals to subspecialists, and other needed services.
Provide instruction and education to help participants self-manage chronic, pregnancy-induced or pregnancy related medical conditions.
Provide information on medication, side effects, risks of nopliance.
Conduct necessary follow up calls with participants to obtain any needed information, remind them of appointments and provide additional education.
Complete all necessary programmatic andanization forms, documents and reports in a timely manner.
Participate in proactive team efforts to achieve departmental andpany goals.
Provide leadership to others through example and sharing of knowledge/skills.
Follow allanization’s policies, practices, and procedures.
Required Skills/Abilities:
Ability tomunicate with a range of constituents, prioritize work effectively in a fast-paced, deadline-oriented office environment. Excellent interpersonal and writing skills
Reliable transportation with proof of valid driver’s license and auto insurance or the ability and means to travel as required.
Experience working with Community Based programs preferred.
Bilingual (English/Spanish) required for some positions
Strong analytical and problem-solving skills.
Ability to prioritize tasks and to delegate them when appropriate
Ability to function well in a high-paced and at times stressful environment
Proficient with Microsoft Office Suite or related software..
Education and Experience:
Bachelor’s degree in Public Health, Social Work or related field with 2-3 years in maternal and child heath, public health, behavioral health, pediatrics, or other fields or Master’s degree in Public Health, Social Work, Behavioral Health or related field with 1 year in maternal and child heath, public health, behavioral health, pediatrics, or other fields.
Proficient personalputer experience including Microsoft Outlook, Word, Microsoft Access, Excel, PowerPoint and Internet applications.
