Job Summary:
The Director of Diabetes Services will work in conjunction with the Population Health Leaders, Physicians and other Nursing leaders to develop and implement best practice standards and programs aimed at improving clinical outcomes for the diabetic patient population within Wellstar Health System. The Director is responsible for the planning across patient care continuum, implementation and monitoring of interventions to achieve this goal. The Director will lead a team that may include Endocrinologists, Certified Diabetic Educators, Pharmacists, Nurses and Coordinators to achieve success through measurable outcomes which includes Diabetes inpatient and ambulatory measures. The initial focus will be to build out the diabetes program to incorporate community health needs assessment as it relates to chronic conditions impacted by Diabetes. Work with the key leaders to build hot spotting maps for DM populations and design specific programs to target population based on community demographics. Emphasis on PCP and specialist education and engagement. The Director will have clinical responsibilities and administrative duties related to population health for WHS diabetics across the Acute Care and Ambulatory Services areas.
Primary Responsibilities:
Implements strategic planning and operational implementation for the Diabetes Program. Analyzes population health data to evaluate effectiveness of the Diabetes Program and identify professional practice and develop opportunities to improve the effectiveness of the program. Works with medial directors to provides educational guidance to PCPs to promote the goals of the Diabetes Program Provides coaching and mentoring to the Diabetes Team to maximize operational effectiveness. Ensures that the Diabetes Program is engaged in community Health programs and Education to raise awareness and drive prevention activities across the service area. Supports the development of a culture of collaboration and excellence both in patient care and provider satisfaction Works to ensure there is adequate endocrinology access for patients and timely communication back to the PCP by the diabetes team Supports shared governance or shared decision making; manage the design and delivery of care that in based on evidence based practice and focused on continuous quality improvement and safety. Requirements It is expected that all nurse leaders are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the patient services team, it is expected the individual upholds the voice of the patient, system policies and procedures while supporting service excellence goals.
Core Responsibilities and Essential Functions:
Directs System operations for Diabetes Disease Management, Diabetes Education, and Diabetes Related Clinical Initiatives.
- Plans, organizes, coordinates, and evaluates system activities, utilizing an interdisciplinary approach
- Serves, along with the Medical Director, as Co-Chair of the Diabetes Advisory Committee
- Provides leadership and direction to the Target Diabetes Clinical Initiatives and Task Forces
- Oversees planning, implementation and evaluation of the Diabetes Self-Management Education entities throughout the System
- Responsible for the development, implementation and compliance to department policies and procedures
- Represents WellStar by providing leadership for various community groups and events
- Identifies opportunities for new program services Ensure that Diabetes Disease Management and Diabetes Education programs and services meet or exceed quality standards
- Responsible for the direction, implementation of and ongoing compliance with the standards necessary to maintain TJC Disease Specific Care certification for all certified programs. Ensures ongoing Survey readiness.
- Serves as the Diabetes Education Program Coordinator and is responsible for implementing standards of practice for maintaining successful ADA Recognition for Outpatient Diabetes Self-Management Education classes.
- Serves as resource to Disease Management, Clinical Support, Diabetes Education and Medical Management staff, physicians, and administrative staff, other disciplines and community to achieve heath care quality goals
- Reviews CORE measure results System-Wide and those applicable to Disease Management Programs, identifying opportunities for improvement. Data oversight and reporting; develop in collaboration with Population Health Analytics team system level reporting for ambulatory and inpatient diabetes reporting
- Assures data integrity for program reporting
- Provides oversight and accountability for development of reports to various System committees, TJC, ADA, VHA and others as applicable
- Presents data reports to various System committees as applicable; monitors monthly and identifies goals and areas of opportunity based on performance Provides Fiscal Management for Diabetes Disease Management and Diabetes Education departments
- Prepares annual operational and capital budgets for Diabetes Disease Management and Diabetes Education (including Target Diabetes) departments
- Monitors financial performance on a monthly basis to include expenditures and variances
- Monitors revenue and class attendance for Diabetes Education classes, identifying opportunities Human Resources Management
- Responsible for oversight and direction of all HR aspects of all personnel in Diabetes Disease Management and Diabetes Education departments
- Directs and supervises the work of department manager to ensure completion of 90 day action plans and other plans as required
- Develops, reviews and revises job descriptions as appropriate
- Serves as a mentor in the professional development of department manager staff
- Reviews all 60-day and annual performance evaluations, progressive disciplinary actions and reviews annual merit requests Performs other duties as assigned Complies with all WellStar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
Baccalaureate degree in nursing (BSN) from accredited school of nursing Required and Master’s Degree in healthcare related field Required Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. Reg Nurse (Single State) or RN – Multi-state Compact Additional License(s) and Certification(s):
Required Minimum Experience:
Minimum 7 years as a Registered Nurse or Clinical Dietitian Required and Minimum 2 years of progressive operational leadership experience in a management role Required and Experience in project management
Preferred Required Minimum Skills:

Ability to think strategically and globally to develop a center of excellence for the diabetes program.
