Provides diabetes self management training and Medical Nutrition Training for patients and their families in accordance with American Diabetes Association Recognition standards including clinical and behavioral assessment education and outcome measurement for individuals and groups. Collaborates with and is an educational resource for the clinic multidisciplinary team in order to enhance quality outcomes. Documents and bills for service within the electronic medical record. Promotes wellness by participating in Diabetes education in the community.

Job Requirements Applicable Experience:
