Full Job Description Job Summary:

The Health Educator will coordinate project activities, provide patient education, conduct chart reviews, and follow-up at the Community Health Center of Buffalo, Inc. (CHCB). In this role, you will develop and implement culturally tailored educational programs with community-based organizations serving low-income underserved populations. Responsibilities:

Educate patients on cancer screening, treatment, and follow-up importance. Conduct chart reviews to identify care gaps and recommend solutions. Develop and implement programs to increase cancer screenings, with a focus on colorectal, cervical, and breast cancers. Participate in quality improvement activities and evaluate care quality. Collaborate effectively with a multidisciplinary team. Communicate efficiently with patients, families, and team members. Coordinate activities to enhance cancer screening with the interdisciplinary team. Ensure program outcomes align with goals and address any variances. Perform other relevant duties as assigned.

Qualifications:

New York State RN License with a minimum of 3 years of experience in a primary care setting. Current CPR certification. Experience in coordinating and providing educational programs to patients. Ability to foster positive business relationships. Proficiency in collaborating with various colleagues and functioning within a diverse care team. Computer literacy, specifically in Microsoft Office.

Physical Demands:

The role involves sitting, standing, walking, bending, and stooping. Some evening work may be required.

$68,000 per year
