45-bed co-ed community facility
Population:
High-risk Responsibilities:
Provide case management and develop individual support plans Assist with daily living skills and crisis intervention Accompany residents to medical/psychiatric appointments Facilitate community integration activities
Qualifications:
2+ years of experience working with the Behavioral Health population Associate’s Degree Solid assessment and documentation skills
Desired Skills:
Bachelor’s Degree Bilingual (English and Spanish) Experience working with the Mental Illness, Substance Use, and Homelessness populations
