Summary:
The primary role is that of a patient educator. Position includes instruction of diabetes self-management training curricula, providing one-on-one diabetes and/or nutrition assessment and training. Position requires thoroughpletion of all materials used in documentation. A diabetes educator provides services with respect for the uniqueness, dignity and autonomy of everyone as stated in the ADCES Scope of Practice and Standards of Practice for Diabetes Educators.
Essential Duties and Responsibilities:
1. Diabetes Education
Utilizes evidence-based guidelines in individual and group teaching situations.
Applies knowledge of principles of teaching and learning to provide education that is age and culturally appropriate.
Demonstrates critical thinking skills in assessment of patients and the development of care plans
Completes all chart inclusions related to patient documentation and physician follow-up in a thorough, clinically appropriate, and timely manner.
Communicates with physicians and other healthcare team members as required or needed according to department policies.
Demonstrates proficiency with operation and download of blood glucose meters, continuous glucose monitors, and insulin pumps.
Attends and actively participates in staff meetings.
Assists in CQI activities and projects assigned.
Serves as role model for other health professionals or students. Supervises student interns as assigned.
Assists withmunity outreach needs as assigned.
Assists in the development and maintenance of patient education materials and curriculum as requested.
2. Professional Development
Maintains memberships in related professionalanizations to demonstrate amitment to professional growth and to stay up to date in the profession.
Completes at least 15 hours annually of continuing education in diabetes care/management.
RD Diabetes Educators will also maintain continuing education requirements for Commission on Dietetic Registration learning portfolio.
Education Training and Experience
Associate’s degree in nursing or Bachelor of Science in Nutrition, Nursing or related health care field (Required)
Master’s degree (Preferred)
Three (3) years or more experience in diabetes education and outpatient counseling (Preferred)
Licensure/Certification
PA State Licensure Dietitian (LDN) OR Registered Nurse (RN) OR Licensed Practical Nursing (LPN) (Required)
Certified Diabetic Educator (CDCES) or in the process of achieving credential within 18 months. We believe that the best care for our patients starts with the best care for our employees.
