Growing together. In this role a Bilingual Lifestyle Coach providesbehavioral change methods through telephonic and chat coaching. Thepopulation group at Level2 consists of members diagnosed with Type2diabetes who are desiring to reverse the effects of diabetesthrough the use of CGMs (Continuous Glucose Monitors), specializedapps and other resources. The Coaches are responsible forcollaborating with a Care Team composed of Level2 Providers,Nurses, Registered Dietitians and other support areas. Level2 is aCare Team focused on areas such as nutrition, physical activity,mindfulness and medication remediation with its members. The coachwill personalize the program to the consumer to activate outcomesprogression. The Coach will meet with members based on member needand risk related priority levels. The Coach will review such thingsas CGM data, exercise data, nutrition information, etc. in order tosupport the member in to moderate their glucose levels. The Coachwill support each member in areas of behavioral change to ensurethe member has the tools and information through technology toachieve their goals. Primary Responsibilities:

• Virtually complete 15-30minute telephonic/chat coaching interactions
• Primary focus ontelephonic engagement
• Behavioral change management of diversemembers (with Type 2 diabetes)
• Ability to empathize, motivate,and encourage all members towards healthy changes Candidates mustbe available to work Monday
• Friday 8:00 am
• 5:00 pm CST (3days/week) and 11:00 am-7:00 pm CST (2 days /week) You’ll berewarded and recognized for your performance in an environment thatwill challenge you and give you clear direction on what it takes tosucceed in your role as well as provide development for other rolesyou may be interested in.

Required Qualifications:

• National BoardCertified Health and Wellness Coach (NBC-HWC) or other CoachingCertification
• 1 years of Coaching/Behavior Change experience withproven results
• Experience working with groups and individualsvirtually (telephonic, etc.)
• Experience working in healthcareenvironment regulated by HIPAA
• Proficient in Microsoft Word,Excel and PowerPoint
• Bilingual Spanish
• Proven solid written andverbal communications skills

Preferred Qualifications:

• Additionalprofessional certifications in health/wellness
• Experience inusing Sales Force, telephony systems and CRM software
• Allemployees working remotely will be required to adhere toUnitedHealth Group’s Telecommuter Policy The hourly range for thisrole is $23.

70 to $46.35 per hour based on full-time employment.

Pay is based on several factors including but not limited to locallabor markets, education, work experience, certifications, etc.
