Affinity is currently looking for a Population Health Tech Medical Assistant (MA) to work in our Population Health Department with availability to travel for patient home visits or providers offices. Population Health Department also offers a quarterly incentive bonus plan.
Job Summary:
The Population Health Medical Assistant will perform a wide variety of duties in the Population Health Program. Primary concentration will be on patient interaction and assisting the Nurse Practitioner/Physician Assistant (NP/PA) with Population Health Visits in the home, provider offices, or via Telehealth. The candidate will have strong phone communication and computer skills. Concentration may also include follow up on patient care, scheduling Population Health Visits, and assisting with Population Health initiatives.
Essential Duties & Responsibilities:
Gather patient information such as medical/surgical history, family history, risk factors, vital signs, and height/weight during the Population Health Visit and/or via telephone prior to the visit Perform screenings during the visit such as IRIS eye exam, PVD screening, HgbA1C, urine microalbumin, etc. Travel with providers in clinics and to patient homes to assist with triaging and EMR (Electronical Medical Records) documentation May include traveling alone to patients’ homes to perform hybrid Population Health Visits. Hybrid visit duties include taking vital signs, performing screenings, gathering patient information, documenting in the EMR, and connecting the patient to a Population Health NP/PA via Affinity Health Group’s (AHG) approved live video platform for completion of the visit Calling to schedule Population Health Visits Provide patient education regarding importance of preventive screenings and other healthcare needs that may arise with the visit Coordinate and schedule screenings for patients such as mammogram, colonoscopy, provider appointments, etc. Coordinate patient care with needed resources such as Integrated Care Management, Social Services, Case Management, Disease Management, etc. Provide follow up phone calls for AHG patients to improve healthcare outcomes. May include patients due for services, follow up from walk-in clinic, and patients needing to establish a primary care provider Assist with Health Fairs Run reports through V-Net to help determine appointment needs Update statuses in V-Net after visit completion Prepare NP/PA folders for home visits or provider office visits Pull claims/diagnoses in V-Net for NP/PA Access, collect, and transfer member information from multiple computer data systems Faxing, mailing, and scanning multiple documents Answering and filtering phone calls Checking for incoming faxes Documentation in Excel, V-Net, and EMR Review and assist with incomplete orders process Float from HRA/APH as needed Coordinate care for SNP (Special Needs Plan) members between the Population Health department, SNP case managers, and any other affiliated resources Assist with any new Population Health program initiatives that may arise
Marginal Duties:
• Other duties as assigned Licenses, certifications, and/or registrations: Certified Medical Assistant certification preferred Current and unencumbered Louisiana Driver’s License Proof of current automobile liability insurance
Knowledge, Skills, and Abilities:
