The ideal candidate will play a crucial role in providing high-quality patient care, assisting with medical records, and supporting various healthcare operations. This position requires a strong understanding of patient needs, particularly in areas such as hospice care and dementia care, while ensuring that all procedures align with established protocols. Responsibilities
- Provide direct patient care and support, including monitoring vital signs and assisting with daily living activities.
- Maintain accurate medical records and documentation using electronic health record systems such as Epic.
- Assist in the development and implementation of care plans tailored to individual patient needs, particularly for home community care settings.
- Collaborate with healthcare professionals to ensure comprehensive patient assessments and interventions are performed effectively.
- Support patients and families in hospice care settings, offering emotional support and guidance during challenging times.
- Ensure compliance with safety protocols and maintain a clean and organized work environment within the medical office or home care setting.
- Participate in ongoing training and professional development to enhance skills related to patient care and medical technology. Experience
- Previous experience in a healthcare setting is preferred, particularly in roles involving medical records management or direct patient interaction.
- Familiarity with hospice care practices and dementia care strategies is highly desirable.
- Proficiency in using electronic health record systems, especially Epic, is an asset.
- Strong communication skills to effectively interact with patients, families, and healthcare team members.
