Overview:
Compensation $18.08-$29.53/hr depending on previous work experience. Thisdepartment is where successful use of data and evidence basednursing care meets quality improvement, and practicetransformation. Our department includes Clinic Care Coordinatorswho work directly with Norman Regional Clinics and their patientsto provide an enhanced level of health promotion, carecoordination, and disease management. We enjoy a collaborativeprocess with both clinic based staff, acute care staff, andpost-acute providers to ensure continuity of care, so our patientscan truly consider Norman Regional Health System their medicalhome. The Care Coordination Health Partner see’s patients in theclinic and/or via phone conference to assist patients with healthcare navigation needs.
Responsibilities:
Job Summary Collaborates with Care Coordinators as well as Providers toassist with coordination of care with multiple external agencies tobest serve the needs of the patient. Act as a patient advocate and liaison between the patient,caregivers, healthcare team and community service agencies. Keep abreast of current trends in the field and integrateknowledge gained into current work practice, with support of CareCoordination leadership. Utilize motivational interviewing skills and culturallysensitive methods to collaborate with patients. Maintains high degree of communication with Care Coordinatorsand other clinical team members to assist in ongoing carecoordination needs and communicate/remove barriers. Planning/Organizing/Initiating Participates and supports departmental quality improvementinitiatives by sharing ideas and experiences and is willing toencourage others to do the same. Provide exemplary customer service skills both with patients,caregivers and community partners.
Qualifications:
Qualifications Customer service experience preferred. Bilingual preferred but not required. Basic computer skills. Excellent communication both oral and written. Highly organizedand self-motivated. Education Bachelor’s degree in any field. In lieu of degree will accept 4 years customer serviceexperience or experience in the insurance/social work/medicalfields. Experience Two years customer service experience or experience in theinsurance field/social work/medical fields. (Above requirements canbe met by the equivalent combination of education and experience).Licensure/Certification The American Heart Association’s Family and Friends CPR and/orBasic Life Support (BLS) training or retraining is required duringorientation prior to providing patient care. CPR course and training must be maintained for the duration ofemployment.
