JOB SUMMARRY

Under the direction of the Patient Relations Manager the Patient Relations Coordinator works to manage Member optimization and VBC patient retention by meeting patient needs and providing necessary services. Works closely with VBC Patient Advocates, ensuring alignment of accurate patient information regarding VBC membership with Medicare Advantage plans (AL and FL). Assists as a resource for Patient Advocates for insurance questions. Works to convert existing patient membership (e.g., Duals), and maintains accuracy of VBC Member Optimization patient list categories: Unattributed, Unassigned, Unmanaged, Drops, Adds, and Duals.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Managing VBC patient calls and assists with the following: Any Medicare patient that is not assigned to a CH provider Medicare Patients Medicare/Medicaid Patients Medicare/Tricare patients Patients turning 65 Medicare patients who are struggling with drug cost/ RX deductible Medicare patients who are unhappy with their current insurance plan or have questions Identifying Medicare patients that could be eligible for Medicaid, Low Income Subsidy, etc.

Daily Duties:

Schedules new patients “First “appointment and Annual Wellness Visit appointments Validates patient insurance for VBC patients and updates in Athena Assess for SDOH within Athena (AHCHRSN) Educates/offer assistance on state’s Medicaid programs Identifies patients for conversion into Value Based Care (VBC) Medicare Advantage and/or Medicaid (Duals) plans by using the “Age-in” report by sending birthday cards Participate in patient enrollment periods and works with patient: AEP (Annual Election Period) October 15th -December 7th. A set time each year that allows Medicare patients to elect their Medicare Coverage if they choose to do so. OEP (Open Enrollment Period) January 1st – March 31st. A set time when Medicare patients have the option of returning to their original plan or enrolling in a Five Star plan, Chronic (C-SNP). ROY (Rest of Year) April 1st – October 15th A set time focus on retention of patients and conversions due to age etc. Works closely with the VBC team Works closely with Patient Access Center and manages calls accordingly Works closely with clinics and helps patients with insurance questions Participates in pertinent meetings and workshops as directed. Interacts with respect and in a professional manner with patients, staff, and external customers Follows HIPAA and OSHA Standards and guidelines. Maintains HR compliance and procedures. Ensures patient satisfaction by providing excellent service putting “patients first always” Other duties as assigned

KNOWLEDGE/SKILLS/ABILITIES

The Patient Relations Coordinator must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability required. A general knowledge of primary care practice, clinics, hospitals, specialists, and ancillary health services and basic knowledge of legal and ethical standards for the delivery of primary care and medical terminology. Must have personal effectiveness and credibility, collaborative skills, communication proficiency and flexibility. Must be able to work independently and as a team player. Uses reason and speaks clearly, respectfully, and persuasively in positive and negative situations, focusing on resolving conflict. Must demonstrate strong decision making and documentation skills.

EDUCATION AND EXPERIENCE REQUIREMENTS

High school diploma or a GED is required Minimum of 2 years medical clinic front office and/or medical assistant experience is required; or equivalent combination of education and experience Strong computer skills and proficiency in internet software and knowledge of Microsoft Office products is required Able to effectively utilize an electronic health record Basic knowledge of Medical Insurance is preferred Basic knowledge of Medicare plans including Medicare Advantage (MA)

HMO, PPO, C-SNP, D-SNP

; Duals) is preferred Athena EHR experience preferred

WORKING ENVIRONMENT

The position requires climbing, stooping, kneeling, crouching, reaching, standing, lifting, grasping, feeling, talking, hearing, repetitive motions, and finger use. Pushing and pulling are occasionally required. Use of a computer, keyboard, and telephone along with various office machines is an essential part of the job.
