The Director, Case Management shall be responsible to direct and supervise the provision of clinical care coordination for all age groups. This individual will direct and supervise the various care coordination functions, including but not limited to utilization review, discharge planning, patient accommodation coordination, and denials management. The Director will monitor and manage employee performance and fosters interdepartmental communication and education regarding case management activities and roles. Additionally, the Director will work collaboratively with the Case Management team, Medical Staff, and Administration to achieve system/facility targets for LOS and resource utilization while being acutely aware of facilitating optimal patient outcomes and quality of care. Qualifications Bachelor’s degree in Nursing required, Master’s degree preferred. At least 10 years hospital-based experience as a case manager preferred. Supervisory experience required. Knowledge of community health care and socioeconomic resources, and third-party payer desired. Knowledge of Joint Commission and CMS Conditions of Participation preferred. Active RN licensure required; Case Management certification preferred.
